No, you don't need to add anything to the request. You might be asked to abide by some kind of edit restriction if you're unblocked, and it's technically possible to leave you partially blocked, but those kinds of things would be done on a case-by-case basis. Jaed Ali is the older account, and the one with more edits, so as we define these terms here this is the sockpuppet account. I don't have a problem with unblocking you, as far as I can see all you are guilty of is a certain amount of messing about five years ago, but I'd rather unblock the original (older) account rather than this one - do you still have access to it? No. Though this account has been blocked as sockpuppet, this is my original account and my original name with Google account I attached. I have no access in Jaed Ali account, I have no account named 'Jaed Ali' in other sites. So, if Jaed Ali account gets unblocked, I may never access in Wikipedia. I don't see anything particularly egregious in their history from 5 years ago, just some messing about that wasted people's time which they've apologised for.
