The pharmaceutical company Amedra increased the price after purchasing the rights to the drug, instead of lowering it as generics are predicted to do, drawing criticism from patients' rights advocates and US Democratic Party leaders.[4]

[edit]In countries with national health care systems (such as the UK and Canada), many of those national health services have restricted bevacizumab on the basis of cost-benefit calculations; in the U.K., for example, the National Institute for Health and Care Excellence has taken the position that bevacizumab should not be funded by the NHS because it costs nearly £21,000 per patient but only minimal benefit in many cancers.[3] In 2006, the Scottish Medicines Consortium recommended against the NHS funding Avastin for first-line treatment of metastatic carcinoma of the colon or rectum, due to estimated costs of £24,000 to £93,000 per quality-adjusted life year (QALY).[4] The addition of bevacizumab to standard treatment can prolong the lives of lung cancer patients by several months, at a cost of $100,000 a year in the United States.[5] For colorectal cancer, Robert J. Mayer wrote in the New England Journal of Medicine that bevacizumab extended life by 4.7 months (20.3 months vs. 15.6 months) in the initial study, at a cost of $42,800 to $55,000.[6] Costs in other countries vary; in Canada it is reported to cost $40,000 CAD per year.[7] On September 16, 2014 Genentech reclassified bevacizumab as a specialty drug which are only available through specialty pharmacies. "Specialty drugs usually fall under the FDA’s Risk Evaluation and Mitigation Strategy (REMS) program, established for compounds like the testosterone... that may have unusual side effects; or for drugs that are unusually expensive."[8] This has caused concern to hospitals as the price increased.[8] According to IMS Health, the average price charged by hospitals for bevacizumab is approximately $9000 compared to approximately $2300 when administered in a doctor's office. As a result of the new distribution arrangement, many hospitals will no longer be eligible for the 51% discount to average wholesale price that was mandated by the Affordable Healthcare Act under the old distribution arrangement.[9] When bevacizumab is used in the treatment of wet age-related macular degeneration (wet AMD), only tiny and relatively inexpensive doses (compared to amounts used in colon and other cancers) are required. Some investigators reported in 2010 that bevacizumab at a cost of around $42 a dose is as effective as ranibizumab at a cost of over $1,593 a dose.[10][11]

[edit]Bicalutamide is off-patent and available as a generic, and its cost is low in comparison to a number of other similar medications (from US$10 to US$15.44 for a 30-day supply of once-daily 50 mg tablets).[3][2] Brand name Casodex costs US$556.17 for a 30-day supply of once-daily 50 mg tablets as of 2017[update].[4] Unlike bicalutamide, the newer NSAA enzalutamide is still on-patent, and for this reason, is considerably more expensive in comparison (US$7,450 for a 30-day supply as of 2015[update]).[5] Bicalutamide is considerably less costly than GnRH analogues, which, in spite of some having been off-patent many years, have been reported (in 2013) to typically cost US$10,000–$15,000 per year (or about US$1,000 per month) of treatment.[6][7]

The foundation now will charge $1,050 for 30 capsules, twice what it charged before".[2]

As of 2018[update] Thailand will produce efavirenz domestically. Its Government Pharmaceutical Organization product costs 180 baht per bottle of thirty 600 mg tablets. The imported version in Thailand retails for more than 1,000 baht per bottle. GPO will devote 2.5 percent of its manufacturing capacity to make 42 million efavirenz pills in 2018, allowing it to serve export markets as well as domestic. Due to price increases by drug manufacturers, Medicaid paid on average $2,685,330 more for Erythromycin in 2016 compared to 2015 (not including rebates).[5] By 2018, generic drug prices had climbed another 5% on average.[6]

Ivermectin is more effective than albendazole and equally as effective as thiabendazole for treatment of threadworm (strongyloidiasis). Ivermectin has fewer adverse effects than does thiabendazole and is at least as well tolerated as albendazole.[4] An analysis based on an economic model suggests that it is cost effective for people moving to Europe from areas where threadworm is common to be given a single-dose of ivermectin on arrival so as to cure presumptive infection with threadworm.[5] Persons who are immunocompromised or who will receive immunosuppressive treatment and who have confirmed or presumptive threadworm infestation are likely to benefit from treatment with ivermectin.[6]

He defended Martin Shkreli, and said, "This is a capitalist economy and if you can’t make money you can’t stay in business." Mulye also attacked the U.S. Food and Drug Administration as "incompetent and corrupt." There is a shortage of liquid nitrofurantoin, because the FDA implemented new rules on impurities, he said. Mulye dismissed the new rules on impurities as a "piece of nonsense."[3] FDA commissioner Scott Gottlieb tweeted a response: "[T]here’s no moral imperative to price gouge and take advantage of patients. FDA will continue to promote competition so speculators and those with no regard to public health consequences can’t take advantage of patients who need medicine."[4]

In November 2018 the Supreme Court of the United Kingdom ruled that Pfizer's second patent on the drug, for treatment of pain, was invalid because there was a lack of evidence for the conditions it covered – central and peripheral neuropathic pain. From October 2015 GPs were forced to change people from generic pregabalin to branded until the second patent ran out in July 2017. This cost the NHS £502 million.[4]

[edit]Trastuzumab costs about US$70,000 for a full course of treatment,[3] Trastuzumab brought in $327 million in revenue for Genentech in the fourth quarter of 2007.[citation needed] Australia has negotiated a lower price of A$50,000 per course of treatment.[4] Since October 2006, trastuzumab has been made available for Australian women and men with early-stage breast cancer via the Pharmaceutical Benefits Scheme. This is estimated to cost the country over A$470 million for 4–5 years supply of the drug.[5] Roche has changed the trade name of the drug and has re-introduced an affordable version of the same in the Indian market.[when?] The new drug named Herclon would cost approximately RS75,000 INR (US$1,200) [clarification needed] in the Indian market.[citation needed] On September 16, 2014, Genentech notified hospitals in the United States that, as of October, trastuzumab could only be purchased through their selected specialty drugs distributors not through the usual general line wholesalers. By being forced to purchase through specialty pharmacies, hospitals lost rebates from the big wholesalers and the ability to negotiate cost-minus discounts with their wholesalers.[6]
