[edit]I removed the following statement today: "(But this wasn't the only fatality; there were many other deaths over the years.)", which had been tagged for needing a reference/citation since Feb/07 - over 20 months ago. The word "many" is a weasel word in this context - how many is "many"? Two? Five? Ten? Forty-two? I doubt that there were more than two or three deaths, but probably several more injuries.
