- Population health management is an approach to healthcare that aims to improve the health of entire populations, rather than just treating individual patients. This can include identifying and addressing the underlying social and economic factors that influence health, as well as implementing policies and programs to promote healthy behaviors and prevent disease. It also includes using data and technology to track and improve health outcomes for a specific population.
