A great start - though it is also commonly called "Reason's Model". I'd check Wikipedia for this to make sure that it doesn't already exist. It would also be great to include a picture to enhance this article. Most sources were good but they weren't all from peer-reviewed journal articles. [edit]the whole article is well organized, but maybe to consider to keep the article with same structure; to using the same format for the Investigation using Reason's model. Such bode the key worlds for point form paragraphs
switch around couple sentence in active failure which explain the concept first then giving examples.so it can be more flow with the rest concepts.
