Album Cover

I am seeking information on legal usage of album covers on Wikipedia. Too often do I found album cover missing or nominated for speedy deletion.
After looking around, reading a lot on wiki, mainly Wikipedia:Non-free content criteria Here is what I am placing for the moment (example It's Not Unusual (album)) (thus most album cover images does not note anything):

Cover of the [group] album [album] (year)
Source: Picture taken using my digital camera by me (Yeg Li) from album (CD) I own. Excerpt from the album (back cover):
* [group] - [album] / Any relevent info
* [any UPC code(whatever they call it on music albums)
* Copyright notice, example: (r)&(c) 1992 I.R.S. records Manufactured by IRS Records 3939 Lankershire Blvc. Universal City, CA 91604.
I Yeg Li hereby make available this picture to the public domain; Please note the ownership of the cover art to someone else (IRS records in 1992) as noted by Wikipedia's Non-Free album cover notice bellow.

Here is a quote I found on Wikipedia image (from an AC/DC album), where most issues are covered, perhaps adding this text would cover it. It is kept here for my future reference.

Fair use in -Album Name-

Though this image is subject to copyright, its use is covered by the U.S. fair use laws because:

1. It's a low resolution copy of a CD album cover.
2. It doesn't limit the copyright owner's rights to sell the CD album in any way, in fact, it may encourage sales.
3. Because of the low resolution, copies could not be used to make illegal copies of the album artwork on another CD.
4. The image is itself a subject of discussion in the article or used in the infobox thereof.
5. The image on the cover is significant because it was made by a famous artist, Artists Name.
6. Derived from a scan of the album cover (creator of this digital version is irrelevant as the copyright in all equivalent images is still held by the same party). Copyright held by the record company or the artist. Claimed as fair use regardless.

The Project's preferred method for this is inline citations using the <ref> tag.

Usage is very simple. Examples:

The citations are displayed by placing a <references/> tag at the bottom of the article.
