In the case of your addition, the proper way to cite the reference would be to simply add the citation at the end of sentence between to ref tags, like : <ref>Mauleón, Rebeca. 101 montunos. Petaluma, CA: Sher Music, 1999.</ref>

You don't need to add "used with permission" because this type of reference is permitted under copyright law. Better still, there is an online reference generator that will produce a properly formatted reference for you to paste into the article.
