The main issue is that the uploader of an image must explain the rationale for why it is believed that the image is fair use in the context in which it is referenced. This rationale should be written so that another Wikipedia editor could make a reasonable inference about what other uses might be allowed or not allowed for the same image in other contexts.
In general, when talking about a movie or television program, it's permissible to use still photographs (especially publicity photographs distributed by the producers for that purpose) or screenshots in order to support and explain relevant points in the article. This means that the image has to be tied into the textual criticism or commentary in some meaningful way, and not used for mere illustration. This is a simple requirement to meet, since there's no way to show what a character looks like without taking a piece of the larger copyrighted work. If the article is a description of the characters, then it makes sense to show each one.
A claim of fair use is weakened as the amount or significance of the small piece increases. For example, a shot of the climactic scene of a movie is more likely to be an infringement than just a head shot of a character, just as reprinting the last few lines of a novel is more likely to be an infringement than reprinting a representative few lines that tie in to a review.
It's also good practice to reduce the size and resolution of the image to that necessary for the particular use for which you are making a claim of fair use, and you should note that in your rationale comment.

Also, you must credit the copyright owner by name, something like "Copyright (c) HBO Television" or whomever, not just by providing a link to the source.
Fair use is inherently fuzzy, so Wikipedia deliberately tries to be considerably more restrictive than the minimum requirements imposed by United States legal concerns.
