Hi Brendon. This entry's off to an excellent start. However, I notice that you don't have a reference for Georges Pearson's 2002 KU dissertation. thanks for the reference John, I will certainly look into it. Unfortunately, I did not notice the discussion tab of my page had been edited until today (too late to include Pearson's work).
