Please note that d/m/y and m/d/y are both acceptable. The order is generally per the use of the initial creation or the country's style. Generally, changing from one order to another should be avoided. m/d/y seems to be primarily American. See wp:Manual of Style/Dates and numbers for a confusing read of style.
