Richard Irvin Cook, MD is a physician and researcher focusing on systems safety, resilience engineering, and expert human performance.

Presently he is the Professor of Healthcare Systems Safety and Chairman of the Department of Patient Safety at the Royal Institute of Technology (Kungliga Tekniska Högskolan) located in Stockholm, Sweden.
