Ohio State University Professor Dr. Christos Yessios and David Kropp formed Auto·Des·Sys in 1990, and the following year released the first version of form·Z. Initially released only for the Macintosh series of computers, a version for Microsoft Windows was released in April 1996. Version 2.9.5 of the software, released in 1998, was the last version that supported 68K Macs and Windows 3.1.

Version 4, released in 2003, added NURBs modeling tools.

Version 5 was released in 2005.
