Aeroflot Flight W-4 was a scheduled passenger flight from Frunze to Leninabad, which on September 3, 1970, the Yakovlev Yak-40 operating the flight crashed on the side of the Mount Airy-Tash. All 21 occupants; which was the first accident of the Yak-40.
Background

[edit]The Yakovlev Yak-40 had the serial number of 9910503 and was registered CCCP-87690. The aircraft was made in 1969, and was equipped with three AI-25 engines, and by 1970, logged 1020 hours in the air and had operated 1344 flights.
Crew
[edit]The crew consisted of Captain Sutormin Vladimir Fedorovich, First Officer Karpov Gennady Vasilyevich, and Flight Engineer Shashkin Vladimir Timofeevich. All of the flight crew were found to have been improperly trained.
Accident
[edit]The Yak-40 departed from Frunze at 20:24PM (LST), with 21 occupants onboard. The weather conditions were clear and the aircraft reached the assigned altitude of 5,700 meters (18,700 ft). The plane suddenly deviated from the path of the flight, which the crew failed to notice and informed the controller that they flew over the city of Namangan and Kokand. At 21:21PM (LST), the tower at Leninabad cleared the crew to descend to 3,300 meters (10,800ft), then told the crew once again to descend to 2,100 meters (6,800 ft), the same attitude as Mount Airy-Tash was. The plane slammed a slope of the mountain and exploded, killing all 21 immediately.
