Appearance
This blocked user's request to be unblocked to request a change in username has been reviewed by an administrator, who declined the request. Other administrators may also review this block, but should not override the decision without a good reason (see the blocking policy). Do not remove this unblock review while you are blocked.

Usage
[edit]Once you have reviewed an unblock request, replace {{unblock-un|requested username|reason for unblock}}
with either:
{{unblock-un reviewed|user=requested username|reason=reason for unblock|decline=decline reason ~~~~}}
{{unblock-un reviewed|user=requested username|reason=reason for unblock|accept=accept reason ~~~~}}
A decline reason should always be included unless the request is an obvious bad-faith use of the template. In such a case, the decline can be written simply as "{{unblock-un reviewed|requested username|reason for unblock}}
".
