The Youth Outcome Questionnaire is a tool used to evaluate youth treatment and therapy programs. It is cited as a reference in many outcome studies and within the body of various Wikipedia articles. You my friend are very quick to nominate pages for speedy deletion, all this occurred within the span of about an hour. I was in the process of editing the article when you tagged it, and I had to recreate some content that was lost. I am removing the Notability and primary source templates. The US National Library of Medicine and National Institutes of Health (see reference 3) is hardly a primary source. Furthermore notability is clearly established by the widespread employing of the questionnaire in the field of psychometrics and by the research done at BYU.
