The given reference [1] points to a page on atrial fibrillation (which is not mentioned here) and does not mention SHD at all. The book itself mentions SHD only in the context of heart failure (pp. 609f). Maybe those pages were supposed to be the reference? Incorrect information

Structural heart disease is defined as disease or dysfunction of heart structures, such as valves, septal defects or cardiomyopathy. It explicitly excludes coronary artery disease.
