Sources and availability of this drug

I went looking for refs on this old drug and also tried to figure out its status. As far as I can tell no company is marketing this drug anymore -- it appears to be one of these drugs that only hospital or compounding pharmacies can provide, which makes it hard to find up to date information about them. Should really set up a category for these drugs.

We use it fairly commonly in Canada and I have not had issues getting it.

In what form do you get it in Canada and who makes it?

I looked at the FDA website and every company that has had an NDA or ANDA has discontinued marketing. See below (FDA has bad URL-practice for linking) AH! Just saw that Hospira's injectable is still being marketed, and bolded it.
