Regarding the claim that this does not represent a global perspective: Not true. Please see all of the training programs in Europe and Australia that are now clinical training models based on the Vail Conference. In the UK there are now many Doctor of Clinical Psychology programs called D.Clin.Psy. while these Practitioner/Scholar model programs are called a D.Psych. in Australia. (see the page on Doctor of Psychology and the Doctor of Clinical Psychology pages to compare the US model to the British/ Australian model.
