Need Headquarters over Evansville, Indiana

Not enough information to where the headquarters is located at and contact information. All you see is few numbers and a name. Not a complete address or phone number.I and toddler about died due to Metropolitan bus driver taking my lane on my side if the road. Semis in city limits on my left lane and my lane was clear until this Metropolitan bus decided to take my lane on purpose.It was a head on collision.
