Merkelis Petkevičius (Google Books: 73 hits) or Melchior Pietkiewicz (Google Books: 42 hits? The raw count suggests Lithuanian name is more popular. When we look at English sources, by adding Lithuania and Poland, we get 3+8=11 hits for Polish name, and [1]+21=20 for Lithuanian. Worldcat has him as [2].
