Comment. I was only 99.9% sure the "Ivan Gates" referred to in most newspaper articles was this person. It was (very) remotely possible that there were two of the same uncommon name who just happened to be pilots in San Francisco, but then I found an obscure newspaper two newspapers, the Atlanta Constitution and the obscure Napa Daily Journal, that did specify that it was "Ivan R. Yes, it does. "Omit unnecessary or redundant details. For example, it is not necessary to state: New York City, New York, United States when New York City, U.S. conveys essentially the same information more concisely." is a direct quote from the documentation - NYC is specifically called out as an exception to the usual format. That guideline is routinely ignored a fair chunk of the time (perhaps because it is iconsistent to not have both in the birth and death places). From List of people from New York City:
