the stamp with Mann's face on it from EAST Germany, the DDR, not simply "Germany."

East Germany still existed in 1971, and the stamp clearly says "DDR," Deutsche Demokratische Republik. West Germany was the "BRD," Bundesrepublik Deutschland.

I understand this as meaning that while Thomas looked down at his brother's habits, Thomas himself was fascinated by young men.
