Putting this together in a hurry because I have to return the book to the library today! I have listed the 12 forests which form the chapter headings, identified their locations from statements in the text, and cited the relevant pages in hidden comments, as a quick and dirty way of recording the source. Feel free to reformat these as proper references to pages of the book if you think fit. I might come back and list the fairytales, too, but they shouldn't need reference from within the book, and I've photocopied the contents page.
