Technically speaking, a fistulOTomy is the opening of a fistula (a single longitudinal incision along the plane of the fistula) whereas a fistulECTomy is the removal of the fistula with some tissue margin (incision to the sides and completely behind the fistula), perhaps someone with more time and eloquence could clarify.
