The title should almost certainly be in sentence case]], thus: Code of offences and penalties. On the other hand, it's not clear if it should be penalties or punishments, according to this ngrams chart. Data is very meager for both of them, but since around 2000, punishments is more common in English books, for whatever reason. Changing to the American spelling yields even more meager results.
