Webster's Collegiate uses the term as a synonym for thenar eminence as if it had only one construction, but I think this is a pretty clear instance of the term with the thumb-tip sense:

... the blood ... welled up into a perfect sphere, balanced precariously on the ball of my thumb.... I stuck my thumb into my mouth.
