OTRS template

See User:Sphilbrick/OTRS summary, this is a "Case 9b".

The article had only been edited four times on 26 November 2008 by AIUM staff, including this edit which inexplicably added the {{csb-pageincludes}} template implying that the CorenSearchBot had flagged the page for possible copyright issues. About two weeks later, Stifle removed the template and put {{permissionOTRS}} on this talk page. As there were never any images in this article, so I am converting to the proper template for text content permissions. An OTRS volunteer may check my work if desired.
