OakLeaf Medical Network is a network of physician-directed health services. The network provides healthcare services in over 30 communities in northwestern Wisconsin. As of February 2024, the network provides care for over 30,000 primary care patients.[1]. Established in 1994, the network includes 175 medical providers across 40 specialization areas[2]. Oakleaf Medical Network is the largest network of independent physicians and clinics in Western Wisconsin[3]
