He is celebrated for pioneering openly gay-themed films, including The Man in the Lighthouse (Ang Lalake sa Parola, 2007), Antonio's Secret (Ang Lihim ni Antonio, 2008), The Game of Juan’s Life (Ang Laro ng Buhay ni Juan, 2009), Pink Halo-Halo (2010), The Commitment (Kasal, 2014), Memories of Forgetting (2020), and The Revolution Knows No Gender (2021). Key Career Highlights:

Antonio’s Secret (2008) won Best Feature Film at Festival de la Luna (Valencia) and Festival del Mar (Ibiza).
The Game of Juan’s Life (2009) received an Honorable Mention in the Narrative Category at the Chicago Reeling Film Festival.
Pink Halo-Halo (2010), the first feature film in the Bicolano Tigaonon language, won Best Editing and Jury Prize for Film for Children at Cinemalaya, emphasizing the effects of war on Filipino families.
Unfriend (2014) premiered at the Berlinale Panorama Section, gaining international attention for its exploration of social media's dangers. It was distributed globally by Fortissimo Films.
The Commitment (Kasal) (2014) was critically acclaimed at Cinemalaya, winning Best Feature Film, Best Music, and Best Cinematography for its nuanced portrayal of a gay couple's relationship. It also received the Special Jury Prize for Best Film at Gawad Tanglaw.
T.P.O. (2016), addressing domestic violence, earned Best Screenplay and Best Sound at Sinag Maynila.
Tale of the Lost Boys (2017), Altarejos' first international feature, won Best Film at Romanian Gay Film Nights and multiple awards at Sinag Maynila. It was screened at major festivals, including NewFest and the Taiwan International Queer Film Festival. in 2018 The film was adjudged Best Film and Best Editing at Sinag Maynila.
Jino to Mari (2019) swept the FACINE International Film Festival (California, USA), winning numerous awards, including Best Film, Best Director, and Best Actor/Actress. It also won Best Screenplay and Best Actress at Sinag Maynila.
Memories of Forgetting (2020) garnered multiple awards, including Best Editing, Best Production Design, and Best Supporting Actress for Dexter Doria, at Gawad Urian, FAMAS, and Gawad Tanglaw.

· In 2021, Altarejos finally released The Revolution Knows No Gender (Walang Kasarian ang Digmang Bayan ) online on November 30 as a tribute to Andres Bonifacio on National Heroes Day. The film earned him Best Editing at Gawad Tanglaw and at Gawad Urian, where it also garnered multiple nominations, including Best Film, Best Director, and Best Screenplay.

Guardia de Honor (2024), a film on police brutality, premiered at the Moscow International Film Festival and competed at Da Nang and Barcelona Asian Film Festivals.

Other Notable Achievements:

Altarejos directed several television series for GMA and TV5 under the pseudonym Jay Altarejos. His works include the youth-oriented POSH, the long-running Legacy, and the popular dramas My Lover, My Wife and Blusang Itim.
