The 5th parameter is optional. If it is omitted, the millennium category will not be populated; e.g. {{DisestcatCountryCentury|15|16th|15th|17th||Aragon}} will not put the century category into the non-existent parent Category:2nd-millennium disestablishments in Aragon. Consensus at WP:CFD discussions points to not having millennium categories for territories that only existed during one millennium.
Parent categories
The template generates appropriate parent categories. One of these, "Disestablishments in [Foo] by century", is only generated if it exists; if it does not exist, the century category will be placed in "Disestablishments in [Foo]", cf. Category:Establishments in the Burgundian Netherlands (see CFD discussion).
Table of decades and years
A table of decades and years will be produced if the century is 11th or later. This can be suppressed by appending the parameter |no-table=yes
Note that categories for decades and years by country before the 11th century are generally deprecated following a series of CFD discussions in 2017, e.g. Wikipedia:Categories for discussion/Log/2017 October 3.
However, decade categories were not merged for the Roman Empire or Byzantine Empire. To display a list of decade categories (but not years), append the parameter |decades=yes
Change of country name
From 2015 to January 2022, this template took an optional parameter for a new name of the country. That parameter is now obsolete, as the template makes use of category redirects instead.
If the default parent category "Establishments in 'X' by century" is a category redirect page, the template will resolve the category redirect, i.e. it will use the target page with the new name instead. Similarly, it will resolve a redirect on the millennium parent.

The parent categories will be Category:Disestablishments in Myanmar by century and Category:2nd-millennium disestablishments in Myanmar, because the default category names Category:Disestablishments in Burma by century and Category:2nd-millennium disestablishments in Burma are both redirected to the corresponding names using "Myanmar".
To resolve links using redirects at every decade and year when an old country name was in use, use {{DisestcatCountryCentury/resolve}}, e.g. as at Category:20th-century disestablishments in Sri Lanka.
