The college was formed as "Sirajganj Medical College, Sirajganj". Soon, it was renamed to its current title. It was named after Muhammad Mansur Ali, a former renowned Bangladeshi politician. In the 1970 elections, he was elected member of the legislative assembly. At the outbreak of the Bangladesh Liberation War in 1971, Mansur went underground to organize a government in exile. During the Bangladesh Liberation War, Mansur was appointed Minister of Finance for the government in exile, tasked with stabilizing and supporting the financial infrastructure necessary to fuel the independence movement. His role was pivotal in coordinating economic strategies and securing resources under challenging conditions, contributing significantly to the liberation efforts.

In the year 2014–2015, Bangladesh government approved to establish 6 new medical colleges at Sirajganj, Manikganj, Jamalpur, Patuakhali, Tangail and Rangamati with a view to improve the healthcare services throughout the country.
