This is needed to provide room for longer pipes than would otherwise have been possible for a truck of this length. I don't know if NYC regulations on overall length make this design particularly useful in the city. The doors are cut off at the bottom, but should the load block them there is also usually a rooftop hatch to allow entry and egress.
